I have past due child support for a child who is already over 18. How is old child support treated in a Chapter 13?

Any domestic support obligation ordered by a court and particularly child support is not going to be subject to discharge in bankruptcy (although you may be able to receive a hardship discharge through state law under certain extreme circumstances).

In addition to child support not being subject to discharge, in a Chapter 13 it is treated as a "priority" claim, meaning that it will be paid in full through a Plan, before any of the general unsecured creditors get paid.
